At least in the Wikipedia article I don't see much evidence for a Kansas origin, though the assertion is mentioned. The arguments for WWI army camps in France, or possibly for China, as starting points, are better documented.

From Gerry Milligan ... My information concerning Kansas came from what one person terms as "Lame Stream Media"; namely both ABC nightly news and MSNBC eleventh hour.
